This is a Health Planner position for our Drug Overdose Prevention & Linkages to Care Program. 

Starting Salary: $19.29 + 3.5% pay raise upon successful completion of probation. 

Job Duties include: 

Plan and develop population and evidence-based interventions to promote wellness for community members of Clark County. Collaborate with community members through person-centered program planning to overcome implementation barriers. Design the composition of program and public health promotion. Coordinate and evaluate the program, including developing promotional materials and messages. Create or facilitate training curriculums for stakeholders and community partners, as well as training of consumers on program details.
 

Write and assess all reporting and data analysis requirements. Conduct financial and programmatic tracking and reporting, communicate with all funding sources, complete all necessary documentation, and oversee/monitor all related projects/activities. Research new funding opportunities; submit proposals; collect and analyze data; and obtain other supporting documentation as needed.
 

Serve as a resource to the community and other organizations/agencies. Participate in or convene community committees, work events and special projects, and develop materials/presentations. Answer general public health-related inquiries and maintain regular contact with partners. Collaborate with internal agency partners through workgroups, committees, or taskforces to coordinate CCCHD programming or response to an identified health issue.
 

Facilitate communication with internal agency partners through participation in division meetings, agency trainings, exercises, or drills.

Drug Overdose Prevention and Linkages to Care:
•Coordinate and facilitate all harm reduction program activities, including but not limited to tracking and ordering program supplies, providing direct service to clients through referrals to wraparound services, collecting and analyzing program data, altering strategies as needed to better serve at risk populations, and maintaining strong working relationships with community partners. (Will include evening hours).
•Serve as a linkage navigator to assist in filling gaps in the local referral system for those in need of harm reduction services and substance use treatment.
•Provide support and assistance in accomplishing the goals of the Overdose Prevention and Linkages to Care grant.
•Collaborate with community partners to expand the Overdose Quick Response Team efforts in the community. Participate in the Community Response Plan which provides guidance for high numbers of overdose cases, implementing as needed.
•Provide administrative support to the Clark County Substance Abuse Coalition.
•Participate in the Ohio Injury Prevention Partnership and Ohio Prevention Network.
Other:
•Assist other heath planners with grant programs as needed.
