SENIOR SOCIAL WORKER

Description

  • Counsels and assesses the needs of clients and their families who are experiencing complex socio-economic problems or severe family dysfunction
  • Assists clients in developing a social service plan to achieve their goals and objectives
  • Coordinates referrals, services and activities with social service providers
  • Assesses the needs of clients to determine and develop goals, objectives, and social service plans
  • Identifies indicators of progress and barriers to achieving family self-sufficiency and family stability
  • Counsels client’s experiencing multiple or complex socio-economic problems, exhibiting severe family dysfunction, or threat of loss of government assistance or housing
  • Coordinates referrals, services and activities with public schools, community ministries, government agencies, court system, and other social service providers
  • Monitors and documents the progress of client’s achievement of social service plans
  • Assesses the needs of clients and their families
  • Counsels and refers clients for parenting skills, children’s behavior beyond control, truancy, abuse or neglect, and other services when not currently active with the court system
  • Assists community council and neighborhood agencies in planning and implementing special social service programs, community projects or events
  • Attends meetings and conducts neighborhood surveys to obtain and disseminate information, and to encourage participation in available programs and services
  • Assists clients who receive housing assistance in developing an individual plan, becoming job ready, obtaining training, and obtaining and maintaining employment to achieve their goal for becoming self-sufficient
  • Conducts home visits, and monitors and documents the progress of clients through to completion of goals and graduation from the program
  • Assists clients who receive Section 8 assistance in developing an individual plan, obtaining their high school diploma, GED, additional education or training to increase their skills to become job ready, and to obtain and maintain employment
  • Conducts home visits, and documents the progress of clients in achieving their goals of becoming self-sufficient and independent from government assistance
  • Assesses clients’ who receive public assistance to determine their readiness for work; identifies related problems, and schedules clients for training
  • Assists clients and employers in resolving work related problems once employment is achieved
  • Contacts employers concerning job openings, explains program benefits, and solicits businesses to participate in the program
  • Obtains housing in contracted apartment complexes for homeless clients and families waiting for Section 8 qualification and until permanent stable housing is obtained
  • Assesses clients’ eligibility and willingness to participate in and adhere to program guidelines
  • Attends meetings, obtains and disseminates information, and encourages participation in available programs and services
  • Conducts home visits, and assesses the needs of clients to be relocated from airport expansion and noise
  • Obtains hardship waivers from airport to expedite relocation process for those with the greatest need, and coordinates services with client, airport and other agencies or contractors
  • Assesses clients’ financial needs, available resources, and mental and physical limitations to determine program eligibility
  • Counsel clients who are newly ill or injured with a condition that may become chronic or terminal
  • Advocates for the needs of clients when available resources or services are limited
  • Interviews family members of deceased indigent persons to determine eligibility for assistance, and contacts funeral home provider to arrange for publicly funded burials
  • Oversees and monitors operations of group home contract provider to assure compliance with program guidelines; and documents contract violations, failure to comply with policies and procedures, and major incidents involving youth and contract staff
  • Explains group home and program guidelines to youth and their family, and counsels and assists youth in adjusting to circumstances of court ordered alternative placement
  • Reviews youth behavior and adherence to program rules to recommend placement or release
  • Conducts life skills training programs, and schedules and arranges for other agencies to provide enhanced training
  • Interviews participants and their parents to discuss and implement court ordered alternative sentencing program restrictions, to determine schedule of permitted activities, and to explain operation of equipment
  • Collects fees, and enters data into computer to enroll participants; to start, stop or change their monitoring schedule; and retrieves information to document violations
  • Operates motor vehicle, and conducts home, school, or work site visits to monitor participants’ activities and installation or operation of electronic monitoring equipment
  • Contacts school counselor or teachers to discuss student’s behavior, attendance, and academic status
  • Testifies in court and contacts attorneys and judges to discuss implementation and changes to court orders or youth’s participation in program
  • Contacts law enforcement agencies to have youth transported to detention facility
  • Monitors and documents the progress of participants through the completion of the program
  • Assists clients participating in the adult home incarceration program in obtaining additional social services and enrolling in treatment programs
  • Assists clients with locating medical, dental, other health related services; child care; financial, housing and state medical assistance; legal services; educational, vocational or job training; and other social service or community resources
  • Meets regular and punctual attendance standards and any special needs of the position as determined by management, including mandatory overtime and performing related work
  • Bachelor's degree - Social Work
  • Two years of social work experience
  • Must provide one's own source of transportation
  • Must possess and maintain a valid driver's license
  • Must be on 24-hour call
  • Must pass an annual tuberculin test
  • Must be vaccinated for hepatitis B, have provided evidence of immunity, or have signed a declination form within ten days of initial assignment
  • Must wear personal protective equipment as mandated by OSHA
  • Must not be convicted of a felony
  • Misdemeanor convictions will be considered on an individual basis
  • Must submit to a pre-employment drug screening
  • Must submit to a pre-employment polygraph test
  • Equivalency:  Bachelor's degree in Social Sciences, and three years of social work experience
    OR Bachelor's degree, and four years of social work experience
  • Additional education in Social Work, Sociology or Psychology may be substituted for experience
  • Applicants and employees in positions which perform job duties that may require contact with offenders in the custody or supervision of the Department of Juvenile Justice or with youth in the care, custody or supervision of Youth Development
  • Must meet qualifications pursuant to the federal Prison Rape Elimination Act, 28 CFR 115.17 and 115.317, to include periodic post-employment criminal background checks
