Healthcare Partnerships Manager

Position: Full-time, Regular (40 hours per week), Exempt

Part-time applicants will be considered (minimum 28 hours per week)

Reporting to: Assistant Director of Food Access

Position Term: This position is initially funded for an 18 month engagement via a grant. We anticipate the likely extension of this position’s work based on success during this initial term.

About the Position

This is a unique, newly created, grant-funded opportunity that will focus heavily on developing reciprocal working partnerships between Food Club and the healthcare industry, creating new systems and frameworks to achieve shared wins in serving our most vulnerable neighbors. 

The Healthcare Partnerships Manager will work to build partnerships between Food Club’s healthy food access programming and local healthcare providers to explore the work of partnering together to address these needs in our community.  The Healthcare Partnerships Manager will provide leadership and support in the achievement of CAH’s strategic efforts to optimize health outcomes for Food Club members with nutritionally relevant health conditions.  They will build relationships with local healthcare providers, develop new referral pathways, and create a framework to track and share individual and community health outcomes and shared wins. 

An ideal candidate will have experience working within or in collaboration with the healthcare sector, be comfortable navigating the healthcare system, enjoy building mutually beneficial relationships and referral pathways, and have the ability to think strategically and creatively to measure and track shared wins and long-term health outcomes. 

Expected Outcomes

  • First 60-Days in the role: complete a strategic outreach plan to increase our knowledge of local healthcare provider landscape, healthcare systems and partnership opportunities.
  • By September 2024: establish proof of concept for clear, convenient referral pathways, shared metrics and outcomes with at least 2 local providers for pilot engagements.
  • By August 2025: actively engage in reciprocal relationships with healthcare providers with at least 6 providers, scaling the network of healthcare-food access partnerships.

Compensation

The anticipated offer range for this position is $54,000-59,000, based on experience and qualifications. Our team is provided with annual compensation reviews that include opportunities for both cost of living and performance increases, as well as performance-based bonuses.
